IMHO I think that not autorunning any files is meant, but customizing the "Open"-action of attachments.
The web browser Opera (up to v12) did have an interesting feature for downloaded files (on windows):
There the context menu of a downloaded file's list entry has a submenu which is the file's context menu, as shown e.g.
in the explorer, from where one can select the "Open with"-action.
By that, the open-actions are still maintained by the operating system and not by sylpheed.
